High dilution homoeopathy continues to challenge the prevalent concept of medicine for more than two centuries. It is really puzzling how even without original medicinal substances different homoeopathic preparations can differ from one another and cure diseases. The skeptics latch on to this absence and go on branding those cures as placebo effect. But, instead of denying inconvenient facts some open minded scientists are engaged in addressing the homoeopathic challenge, namely, answering two most fundamental questions: (1) How various super-diluted substances differ from one another? (2) What is the clue to their medicinal value vis-à-vis how can they cause cures? This article shows that skeptics’ objections are logically refutable. But, the real necessity is scientific explanation. Thanks to three striking properties of water the cloud seems to be thinning out. These properties are discussed in explaining homoeo-mystery. They answer the first question as: Various super-diluted substances may be chemically just water but they differ from one another in their molecular structures. Another special property of water and template principle of biology lead to the structural model of medicines answering the second question as: A substance is to be recognized as a medicine if it has the capability of curing disease(s) while its medicinal property is to be attributed to molecular structure of vehicle like water or of distinct chemical substance, when it exists. Finally, structural concept in non-homoeopathy, like locking principle of antibiotics and shift of focus from chemical formula to physical structure suggest structures as generalized form of medicine. Background Homoeopathy is the newest art and science of healing. Like convention￾al medicine, pharmacovigilance is an important aspect of homoeopathy. There is a paucity of knowledge and ignorance among the practitioners. The necessity is to create awareness and encourage the homoeopathic professionals regarding drug safety surveillance, documentation and reporting of adverse events. Methods A web-based online search from databases, journals, bibliographic resour￾ces regarding the practice of pharmacovigilance in homoeopathy was done. Articles and publications till January 2021 along with literature from homoeopathic books were analysed. Results Data on adverse drug reactions (ADRs) and events in homoeopathy have been collected and reported in European countries. In India, of late these terminologies are being acknowledged among a group of homoeopathic practitioners only. Still, a dearth of cognizance on the subject exists and little reporting is done. People usually do not relate such adverse reactions to the use of homoeopathic medicines. For scrutinising the same, the Ministry of AYUSH, Govt. of India, has taken an initiative for Pharma￾covigilance of Ayurveda, Siddha, Unani & Homoeopathy systems of medicine for reporting and taking measures against ADR and objectionable advertisements. The probable risk factors and ways to prevent ADR and benefits of pharmacovigilance in homoeopathic practice could be deduced. Conclusion Homoeopathy is a widely followed system of medicine that is quite popular among the common people. The need of knowledge and promotion of suspected ADR reporting should be emphasised to increase the credibility of homoeo￾pathy among the scientific community. Also, the profession should refrain from misleading and objectionable advertisements in print and electronic media.

Introduction: Self-Medication is a practice common in developing countries, and accepted in society, unlike the western world. NSAIDs are responsible for the delayed diagnosis (due to suppression of clinical features), disease progression and adverse effects. Objective: To determine the factors leading to self-administration of NSAIDs, their usage pattern and possible adverse effects. To compare NSAID’s use by prescription and over the counter acquisition. Materials and Methods: The present study investigates the determinants and effects of self-administration of NSAIDs by patients attending a tertiary care hospital during August- October 2019. The patients were interviewed regarding basic socio demographic variables, their rationale behind taking NSAIDs along with adverse effects experienced by them with a validated questionnaire. Their awareness pertaining to the drugs was also assessed.
